This is a Comprehensive Biopsychosocial Assessment, Evaluation, Diagnosis and Recommendations.
1st session is 90 minutes, interview, information gathering appointment where I learn about you and learn about the biological, psychological, and social areas of your life.
It is common practice in SUD evals to have a Collateral Contact. This a person you choose, you would sign a ROI, giving me permission to contact this person. If you are on probation, I do correspond with your probation officer.
You will leave the session knowing my diagnosis, recommendations and next steps. My diagnosis and recommendations could change if significant information is left out, or your collateral contact sources share conflicting information.

Document Review: Review of Drug Screening, Police Reports, CPS reports.
Comprehensive Report
Fee: $275.00 per hour. Retainer fee of $2,200.00 required.
Substance Use Disorder Evaluation, this is not a Rule 25 assessment. I can do Rule 25 assessments if needed.
