Substance Use Disorder Evaluation/ Psychiatric Diagnostic Assessment

I recommend using an independent clinician to complete your evaluation to ensure your evaluation and recommendations are not at risk of a conflict of interest situation that may occur if you complete your SUD evaluation a treatment center.

  • I do not recommend using your health insurance to cover the fee for a SUD evaluation unless you need treatment and will using your insurance to pay for treatment. This offers you privacy and agency over your medical footprint.

  • SUD at Space Psychological Services uses ASAM criteria and DSM-TR 5 criteria to complete your evaluation and determine referrals and/or recommendations.

~ Written Evaluation: A comprehensive BioPsychoSocial summary with recommendation(s).

~I do not accept insurance for SUD evaluations. I recommend keeping evals free from your medical footprint, this means paying out of pocket so this does not become part of your medical records.

What is the difference between a Diagnostic Substance Use Disorder Assessment and a Forensic Substance Use Disorder Assessment?

If you have an extensive mental health, substance use, multiple treatments, hospitalization, arrest, legal history that require additional time to obtain all needed information this may require a forensic assessment.

A forensic assessment may include: review of police reports, review and interpretation of Drug Screen (UA) reports, additional collateral contact (s) are needed.

GOOD FAITH ESTIMATE
