No Surprises Act and Good Faith Estimate

This applies to folx who are private pay or uninsured

 

STANDARD NOTICE

·     
Under the No Surprises Act You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.

 

·      Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

 

·      You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.

This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

 

·      You can also ask your health care provider, and any other provider you choose for a Good Faith Estimate before you schedule an item or service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

 

·      Make sure to save a copy or picture of your Good Faith Estimate.

About Me

It is important to feel you have a connection with your therapist. Here is a bit about me to see if we might work well together. I hold my values as a safety container. I am an advocate for what I believe in.

I am currently practicing 100% via Telehealth. I am in the process of bringing my groups to in-person meetings.

A little bit of info about me…

     I am a non-pathologizing, anti-racist, LGBTQIA+ affirming, creative, somatic, narrative, funny {at least I think I am funny} therapist who is motivated by a social justice lens. I have training in EMDR, Brainspotting and IFS {Internal Family Systems}.

My niche areas are trauma, depression, anxiety/panic, addiction and behavioral addiction: {hypersexual disorders, gambling, shopping, shoplifting, gaming}.

Burnout/vicarious trauma, intimate partner betrayal/trauma and working with professional in HPSP.

(SUD) Substance Use Disorder evaluation

I conduct (SUD) Substance Use Disorder evaluation, also called chemical health assessments, a comprehensive assessment and screening for SUD and co-occurring disorders.

Screening and Assessments for SUD are essential to understand the scope of the issues.

I use DSM TR-5 and ASMA criteria in determining diagnosis.

I am a licensed psychotherapist. I hold a master’s degree in counseling psychology, an undergraduate degree in forensic psychology with a specialization in forensics and the law and an undergraduate degree in addiction counseling.

I am a Licensed Professional Clinical Counselor (LPCC), Alcohol and Drug Counselor (LADC), I hold a certification in Dialectical Behavioral Therapy (C-DBT)
